TERMS OF SERVICE

By booking an appointment, you agree to the following:

  1. You consent to receive treatment and understand what it involves.
  2. You understand that the treatment area will be exposed. Please wear appropriate clothing (e.g., a bra for upper body issues or shorts for lower body issues). Undergarments will not be removed, and non-treated areas will remain covered with towels.
  3. You agree to complete the patient form accurately, including all relevant medical history.
  4. You acknowledge that Revitalise Motion is not a substitute for medical treatment or medication.
  5. You understand that Alex Walsh, the therapist, does not diagnose illnesses or injuries or prescribe medication.
  6. You agree to inform the therapist of any new symptoms, medications, injuries, or discomfort before or during the session.
  7. You understand that treatment will not proceed if contraindications are present.
  8. Clients under 16 must have a parent or guardian present to the sign consent.
  9. Appointments require at least 24 hours’ notice for cancellation or rescheduling. Late rescheduling (less than 24 hours’ notice) incurs a fee of 50% of the appointment cost. No-shows are charged 100%, payable before booking future sessions.
  10. If you are late, please contact us as soon as possible. Arrivals more than 15 minutes late may result in reduced session time or cancellation unless otherwise agreed.
